What is Recurrent Pregnancy Loss (Habitual Abortion)?

Two or more consecutive loss of pregnancy is called recurrent pregnancy loss (or habitual abortion). In 2016, the American Academy of Obstetrics and Gynecology recommended that if you have experienced more than three abortions, you should receive an overall examination of the cause. Approximately less than 5% of women experience two consecutive pregnancy losses, and 1% of women experience more than three.

Possible Causes of Habitual Abortion

Chromosomal Cause

Most pregnancy loss (about 60%) results from an abnormal number of chromosomes during embryo fertilization and it is not linked to any other medical diseases. However, the frequency of chromosomal abnormalities tends to increase for older women pregnancies. In case of couple whose chromosomes are translocated, their oocytes and sperm may have abnormal chromosomes without apparent indications, leading to loss of pregnancy.

Shape of uterus

Abnormal shape of uterus can also be cause of habitual abortions. These include congenital anomaly, endometrial adhesion, fibroids or polyps.

Thrombotic tendency

If you have a thrombotic autoimmune disorder, such as an anti-phospholipid antibody syndrome, you may also develop habitual abortion.

Diabetes

The frequency of miscarriage increases if blood sugar level is not regulated properly.

Polycystic ovary syndrome

Polycystic ovary syndrome affects woman’s hormone levels, periods, and ovulation and increases the ricks of abortion.

Habitual abortion test and treatment

To detect the any of the causes above, we will perform tests such as chromosome tests, blood tests, and ultrasound tests and treatments will be performed accordingly.

For example, if chromosomal abnormalities, such as dislocations, are found, genetic counseling is performed and a pre-implantation genetic test is performed through in vitro fertilization to identify whether normal embryos can be isolated. If a uterine anomaly such as a septum uterus is observed, surgery to remove the septum through the endoscopy can significantly reduce the abortion rate. If an antiphospholipid antibody syndrome is found in a blood test that confirms thrombus propensity, treatment with heparin (thrombolytic agent) and aspirin may be performed simultaneously.
